Medication Assistance Program(MAP)

MAP is a Patient Assistance Program sponsored by pharmaceutical companies to allow uninsured patients to receive certain provider prescribed medications, free of charge for up to a year. Qualifying patients will need to complete an application and pay an processing fee of $10.00 for initial enrollment. Ask your provider for more details.
